Forms - Fond du Lac Area Youth for Christ

Below are some links of forms needed to be filled out in order for you child to participate in the activities that Youth for Christ sponsors. Please print the appropriate forms for the activity your child wants to participate in. For the safety of you child a Medical Release needs to be on file with YFC. It is the parental responsiblity to notify YFC of any changes in medical condition/release. A parental permission form needs to be filled out for each separate activity. Automatic Withdrawal Form

If you would like to financially support Youth for Christ general program, The Union or one of our staff members on a monthly basis. You can fill this form out and send it to our office.
